TERMS & CONDITIONS:

1. To be paid only to former members of Malaysian Nurses Foundation (MNF) on his / her demise.
2. The sum of RM 2,000.00 will be given to beneficiary named as appeared in the nomination form only.
3. In the event that no beneficiary was named, the benevolent will be given to the spouse/ child or immediate family member who cared for the demise.
4. A copy of Death Certificate must be produced before payment.

APPLICATION PROCESS AND SUPPORTING DOCUMENTS

The beneficiary must submit the completed nomination form together with the following documents:
